By electrically signing below, you acknowledge and approve the following statement

In the case of medical emergency, I understand every effort will be made to contact me. In the event I cannot be reached, I hereby give permission to the physician selected by the SnowBlast director to hospitalize, secure proper treatment for, and order injection, anesthesia or surgery for my child. I also certify that my child has no communicable diseases. Also, any pictures, quotes, or video of my son/daughter may be used on SnowBlast promotional material or on Kirby’s church website.
